Anyone who has been in hospital as a patient or at the side of family members there is aware of the many things that can go wrong. There are delays. Steps get missed. Communications break down. The left hand in one department doesn’t seem to know what the right hand in another is doing.
Most of the time, mercifully, these individual errors lead to no more than discomfort, inconvenience and worry.
